Information for making a referral to our Cardiac Surgeons
Please complete the appropriate form enclosed below then fax it to us (paper copies of faxed referrals are not required). If your patient has not heard from our office within one week after faxing a referral, please call our office.
We triage our referrals daily and appointments are given according to the urgency of the patient’s condition. We will contact the patient directly to set up an appointment.
IF we have been unable to contact the patient for an appointment we will fax you back a letter and let you know we have been unable to do so. We will then ask you to contact the patient and instruct them to phone us.
If we feel that the patient does not need to be seen by us we will contact you to let you know why / make a suggestion as to who we feel would be more appropriate.
Cardiac Surgery and Pace Maker procedures are our specialty. We do not do Thoracic / Peripheral Vascular work.
When preparing the referral, it is important to provide us with the most current information that is available. Please include only the most recent relevant consult report from a specialist and relevant non-Island Health laboratory work ( recent CBC / GRF / BNP / If Diabetic HgbA1C within past 90 days ).
PLEASE do not send anything that is available from VIHA's Powerchart as we have full access to it ( i.e. do not send ECHO / CT / Ultrasound / Angiogram / MRI / Xray reports / etc.)
We book patients with our 'First Available Surgeon' model. If you want to refer to a specific Cardiac Surgeon we will be happy to direct it to that physician. Sometimes however we may direct your referral to another of our surgeons if they have a particular expertise in the patient’s condition.

There is a wealth of information enclosed here that will help them prepare for their office visit as well as aid them in understanding their condition prior to seeing the surgeon. We will request that they complete the Patient Questionnaire and bring it with them for their appointment..
